Job summary

Continuum of Care is seeking a Residential Aide for 2nd shift in New Haven to support residents at Jimmys Place. You will help with daily living activities, monitor health and safety, and facilitate group activities with nursing staff oversight.

The role requires a high school diploma (CNA welcome) and reliable transportation. Weekend differential applies, with a pay rate around industry norms for the area. Must be able to work Thursday–Saturday and Sunday–Monday shifts.

Qualifications

• High School Diploma or equivalent; one year healthcare experience preferred.
• CNA or related certification accepted in lieu of experience.
• Connecticut driver’s license with clean driving record required.
• Must maintain reliable transportation for scattered-site/non-24 hour programs.

Responsibilities

• Establish rapport and professional boundaries with clients.
• Promote independent living skills through teaching and prompting.
• Assist with activities of daily living and household tasks.
• Lead psychoeducational groups and community activities.
• Provide transportation to appointments and assist with travel planning.
• Document shift logs and medical-related observations.
