Job summary

Continuum of Care is seeking a Residential Aide for the ISLP program in New Haven. You will assist individuals with daily living activities, provide prompts for independent living, and participate in group activities and outings on a 3rd shift schedule (Sat-Sun 12 am-8 am).

The role requires a high school diploma, CPR/First Aid certification, and a valid Connecticut driver’s license. Weekend shift adds $1/hour differential; base $20.17/hr, weekend $21.17/hr.

Qualifications

• High School Diploma or equivalent required; relevant healthcare experience preferred.
• CPR/First Aid certification required or provided during orientation.
• Proficient computer skills including electronic record keeping and Microsoft Office.

Responsibilities

• Establish rapport and professional boundaries with clients.
• Provide daily living skills coaching and prompt care activities.
• Lead and participate in group activities and community outings.
• Document shift logs and client information in electronic records.
• Assist with medication self-administration monitoring.
• Transport clients to appointments as needed.
