Job summary

Chimes is seeking a dedicated professional to join our Mental Health Program in West Chester, PA. You will provide person-centered services and supports for adults and families within a compassionate nonprofit framework.

Responsibilities include developing service plans, helping residents with life skills, offering counseling and crisis intervention, and connecting clients to local resources, while maintaining thorough documentation in a collaborative team setting.

Qualifications

• Associate's Degree with 1 year of experience in a human service-related field.
• Clearances: Criminal Clearance and Fingerprint-based federal criminal history; Valid Driver’s License.

• Assist residents with life skills development, including budgeting, self-care, medication management, and community living skills.
• Provide supportive counseling and crisis intervention when needed.
• Develop and implement individualized service plans.
• Connect residents with community resources and support services.
• Maintain accurate and timely documentation
• Collaborate with treatment teams, providers, and supports to promote positive outcomes.
