Job summary

Holcomb Behavioral Health Systems - A Chimes Company in West Chester, PA is seeking a dedicated professional for a full-time Mental Health Program role. You will provide person-centered services aligned with Recovery Model principles, assist residents with life skills, and deliver supportive counseling within a collaborative treatment team.

Qualifications

• Associate's Degree with 1 year of experience in a human service-related field.
• Criminal Clearance and Fingerprint-based federal criminal history check required.
• Valid Driver's License required.

Responsibilities

• Provide person-centered services in accordance with Recovery Model principles and program standards.
• Assist residents with life skills development, including budgeting, self-care, medication management, and community living skills.
• Provide supportive counseling and crisis intervention when needed.
• Develop and implement individualized service plans.
• Connect residents with community resources and support services.
• Maintain accurate and timely documentation.
• Collaborate with treatment teams, providers, and supports to promote positive outcomes.

Schedule: Full-Time; Monday through Friday, 9:00am - 5:00pm
