Job summary

Chimes in West Chester, PA is seeking a dedicated professional for the Mental Health Program to provide person-centered services, assist residents with life skills, offer supportive counseling and crisis intervention, and develop individualized service plans. You will connect residents with community resources and maintain accurate documentation in a team-focused setting.

Qualifications

• Associate's Degree in Human Services or related field with 1 year of experience.
• Criminal clearance and fingerprint-based federal background check; valid driver's license.

Responsibilities

• Provide person-centered services in accordance with Recovery Model principles and program standards.
• Assist residents with life skills development, including budgeting, self-care, medication management, and community living skills.
• Provide supportive counseling and crisis intervention when needed.
• Develop and implement individualized service plans.
• Connect residents with community resources and support services.
• Maintain accurate and timely documentation.
• Collaborate with treatment teams, providers, and supports to promote positive outcomes.

Pay Rate: $18.00+/hour
Schedule: Full-Time; Tuesday through Friday, 1:00pm - 9:00pm, Saturday, 8:00am - 4:00pm

What You'll Do
