Job summary

Holcomb Behavioral Health Systems - A Chimes Company in West Chester, PA is seeking a dedicated human services professional for a full-time Mental Health Program role. You will provide person-centered services, support adults and families, and collaborate with treatment teams to promote positive outcomes.

This position requires an Associate's Degree with 1 year of relevant experience, plus clearances and a valid driver's license.

Qualifications

• Associate's Degree with 1 year of experience in a human service-related field.
• Criminal Clearance and Fingerprint-based federal criminal history clearance.
• Valid Driver’s License.

Responsibilities

• Provide person-centered services in accordance with Recovery Model principles.
• Assist residents with life skills development including budgeting, self-care, medication management, and community living skills.
• Provide supportive counseling and crisis intervention when needed.
• Develop and implement individualized service plans.
• Connect residents with community resources and support services.
• Maintain accurate and timely documentation.
• Collaborate with treatment teams, providers, and supports to promote positive outcomes.

Full-Time; Tuesday through Friday, 1:00pm - 9:00pm, Saturday, 8:00am - 4:00pm
