Job summary

The Postgraduate-Center-For-Mental-Heal is seeking a Residential Case Manager to provide supportive counseling, crisis intervention, and documented services for residents. Qualifications

• Bachelor’s degree in a human services field required or 2+ years of equivalent residential case management experience.
• Experience in mental health settings preferred, with strong crisis de-escalation skills.
• Proficient in documenting client progress and maintaining service plans.

Responsibilities

• Work with an assigned caseload to provide supportive services and crisis intervention as needed.
• Develop, implement, and document individual service plans to achieve rehabilitation goals.
• Maintain accurate case records and progress notes for each resident.
• Lead groups and provide counseling, role-modeling, and supportive guidance.
• Coordinate medical appointments, entitlements, and community resource referrals.
• Monitor safety and security of the facility and respond to emergencies promptly.
