Job summary

Postgraduate Center for Mental Health is seeking a Scatter Site Case Manager to provide supportive counseling to residents, help them achieve short- and long-term goals, and intervene during crises as needed. The role includes documenting services and maintaining safety in resident apartments.

You will manage a caseload, conduct assessments across functional, employment, nicotine, and substance-use domains, lead groups, and coordinate with community resources while following PCMH policies.

Qualifications

• Bachelor's Degree in Human Services required.
• Minimum two years of experience as a Case Manager or Counselor in residential, mental health, or related setting.
• An Associate Degree may be substituted for adequate experience (5+ years).

Responsibilities

• Provide support services to residents on assigned caseload.
• Document progress notes, service plans, and records as required by PCMH policies.
• Conduct assessments including Functional, Employment, Nicotine, Substance Abuse, or related evaluations.
• Lead recreational, counseling, and rehabilitation groups.
• Escort clients to medical, benefit, or other pertinent appointments.
• Coordinate and participate in community activities and partnerships.
• Manage crises and de-escalate when needed; notify supervisor and document events.
