Job summary

Postgraduate Center for Mental Health Residential Case Manager role provides supportive counseling to residents, helping them reach long- and short-term goals while delivering crisis intervention as needed.

Responsibilities include maintaining case records, developing service plans, leading groups, coordinating with medical and community resources, and ensuring safety within the facility.

Qualifications

• A bachelor’s degree in a Human Service discipline is required.
• Minimum two years of experience as a case manager or counselor in residential/mental health setting.
• Associate degree with 5+ years of related experience may apply.

Responsibilities

• Work with an assigned caseload of residents to provide support services.
• Implement individual service plans to attain rehabilitation goals.
• Maintain, develop and document accurate case records and progress notes.
• Offer instruction and service delivery across service categories through modeling and guidance.
• Manage crises within the residence and document events timely.
• Order, supervise, and count medication where applicable.
• Provide counseling to clients/residents as required.
• Lead recreational and other rehabilitation groups.
• Escort clients to medical, benefit, or pertinent appointments.
• Coordinate entitlement-related services and housing-related activities.
• Document significant building activities in logs and participate in staff meetings.
• Adhere to policies and procedures and perform other duties as assigned.
