Job summary

Catholic Charities of New York is seeking a Residential Counselor to implement the Community Residence or Permanent Housing Program. You will help residents build life skills aligned with their comprehensive service plan, focusing on customer service, service delivery, and sound judgment during crises.

Qualifications

• High School Diploma.
• Experience in helping individuals with mental illness or similar populations.
• Ability to learn CPR/First Aid and maintain certification.
• Ability to work independently and as part of a team.
• Willingness to work flexible hours and be available for emergencies.

Responsibilities

• Provide case management services to all assigned residents.
• Implement individual service plans to assist residents in attaining rehabilitation goals.
• Offer instruction and service delivery in 11 service categories through role modeling, guidance, training, assistance and support.
• Document all services and interactions in residents’ case records and maintain records.
• Engage residents in discussions about their goals and evaluate the services received.
• Provide ongoing counseling support to residents as needed.
• Draft the resident’s comprehensive service plan and facilitate involvement in the plan.
• Act as liaison for services needed by residents, including scheduling, referrals, and follow-up.
• Manage incidents, minimize risk, and notify the program’s Manager when needed.
• Integrate care with agency staff and other service providers.
• Attend required meetings and assist with maintaining the environment.
• Supervise medication distribution where applicable and assist with meals.
• Escort clients to medical, benefits, or relevant appointments.
• Conduct bedroom/apartment inspections and document activities.
