Job summary

Catholic Charities of New York is seeking a Residential Counselor to support residents in the Community Residence or Permanent Housing Program. You will help residents learn daily living skills, implement service plans, and document services to promote independence and well‑being.

You will provide case management, crisis support, and coordination with medical, social, and legal services while maintaining comprehensive records and contributing to a safe, respectful living environment.

Qualifications

• High School Diploma required.
• Experience with helping individuals with mental illness or similar population is preferred.
• Ability to learn and use program tools and documentation systems.

Responsibilities

• Provide case management services to all assigned residents.
• Implement individual service plans to assist residents in attaining rehabilitation goals.
• Document all services and interactions in residents' case records.
• Engage residents in discussions about goals and progress; provide counseling as needed.
• Assist with planning and execution of recreational activities and daily living skills.
• Coordinate with other services per the comprehensive service plan and liaise with external providers.
• Ensure safety and crisis management, including incident reporting and documentation.
