Catholic Charities of the Archdiocese of New York is seeking a Residential Counselor to support residents in a Community Residence or Permanent Housing Program. You will help residents learn life skills per their service plans, provide and document services, and exercise sound judgment in crisis situations.

Responsibilities include case management, implementing service plans, coordinating with other services, and maintaining accurate documentation.

Qualifications

• High School Diploma or equivalent.
• Experience helping individuals with serious mental illness or similar populations.
• Good judgment and strong customer service skills.
• Ability to work independently and as part of a team.

Responsabilités

• Provide case management services to all assigned residents.
• Implement individual service plans to help residents attain rehabilitation goals.
• Assist residents in daily living activities such as grooming, meal planning, budgeting, and health care.
• Document services and interactions in residents' case records and maintain records.
• Engage residents in discussions about goals and service evaluations.
• Provide ongoing counseling support as needed.
• Draft comprehensive service plans and facilitate resident involvement in planning.
• Coordinate with other services and schedule follow-ups, referrals, and related processes.
• Manage incidents to minimize risk and notify the program manager.
• Integrate resident care with agency staff and external providers.
