Job summary

Catholic Charities of New York is seeking a Residential Counselor to support residents in Community Residence or Permanent Housing programs. You will help residents learn life skills, implement service plans, and document services while exercising sound judgement in crises.

This full-time role requires flexible shifts, the ability to accompany clients to appointments, coordinate with medical, financial, and social services, and maintain comprehensive records.

Qualifications

• High School Diploma or equivalent.
• Experience helping individuals with serious mental illness.
• Strong customer service and communication skills.
• Ability to work flexible hours and be available for emergencies.

Responsibilities

• Provide case management services to assigned residents.
• Implement individual service plans to assist residents in attaining rehabilitation goals.
• Offer instruction and service delivery in service categories through role modeling, guidance, training and support.
• Document all services and interactions in residents’ case records and maintain the records.
• Engage residents in discussions about their goals and evaluate services provided.
• Provide ongoing counseling support to residents as needed.
• Draft the residents’ comprehensive service plan and facilitate involvement in discussions.
• Act as liaison for services needed by residents per the service plan.
• Manage incidents and intervene to minimize risk, documenting events.
• Integrate care with agency staff and other service providers.
• Attend required meetings and assist with maintenance of the living environment.
• Supervise medication distribution where applicable and assist in meals.
• Escort clients to appointments and conduct building inspections.
• Document significant activities and ensure Visitor Log compliance.
• Assist with monthly fire drills and planning recreational activities.
• Follow NYS Office of Mental Health guidelines and internal policies.
