• Experience helping individuals with mental illness or similar population preferred.

Responsibilities

• Provide case management services to all assigned residents.
• Implement individual service plans to assist residents in attaining rehabilitation goals.
• Assist with daily living skills such as grooming, dressing, meal planning, budgeting, and health care.
• Document services and interactions in residents' case records and maintain the records.
• Engage residents in discussions about goals and evaluate services received.
• Provide ongoing counseling support as needed.
• Coordinate with other services and agencies per the service plan.
• Assist with planning and carrying out recreational activities for residents.
• Assist in safety and crisis management and maintain logs.
