Job summary

Catholic Charities of New York is seeking a Residential Counselor to support residents in Community Residence or Permanent Housing programs. You will help clients learn life skills, implement service plans, and document services in records to foster independence.

You will provide counseling, coordinate with medical, housing, and benefits services, and assist in daily activities while adhering to program guidelines and safety protocols.

Qualifications

• High School Diploma is required.
• Experience helping individuals with serious mental illness or similar population is preferred.
• CPR/First Aid training is desirable and may be required.

Responsibilities

• Provide case management services to assigned residents.
• Implement service plans to assist residents in rehabilitation goals.
• Deliver services across 11 categories through role modeling, training and support.
• Document services and interactions in residents' case records.
• Engage residents in discussions about goals and service evaluations.
• Provide ongoing counseling support as needed.
• Draft and facilitate the residents' comprehensive service plan with involvement.
• Coordinate with other services (medical, psychiatric, benefits, housing, etc.).
• Monitor and document incidents, and maintain safety and risk management.
• Attend meetings and help maintain the program environment.
