Qualifications

• High School Diploma required.
• Experience helping individuals with serious mental illness or similar population.

Responsibilities

• Provide case management services to residents.
• Implement individual service plans to aid rehabilitation goals.
• Assist with daily living skills and supervise medication distribution where applicable.
• Document services and interactions in case records and logs.
• Engage residents in goal discussions and counseling as needed.
• Coordinate with services and plan visits, transportation, and appointments.
• Attend required meetings and maintain environment safety and cleanliness.
