Job summary

HumanGood in Bellmawr, NJ seeks Social Services Coordinators who are passionate, organized and compassionate toward residents. You will organize, implement and evaluate the Social Services program to meet psycho-social needs and connect residents with supportive services to continue living independently.

This full-time role runs Monday to Friday, 9:00 AM – 4:00 PM, with pay: $23.00–$25.00 per hour, depending on experience.

Qualifications

• 1+ years of case management experience required.
• 2+ years of social services experience, preferably in a senior care setting.
• Bachelor's degree in social work or related field.
• Social Services certification preferred.

Responsibilities

• Assess resident needs based on admission social services history, health and psychosocial factors.
• Promote a positive living experience for residents.
• Maintain resident records and ensure confidentiality per HIPAA and privacy standards.
• Collaborate with the community team to coordinate supportive services.
