HumanGood is seeking a Social Services Coordinator to organize, implement, and evaluate the Social Services program for residents, ensuring psycho-social needs are met so residents can live independently in the community.

The role is full-time with weekday hours (Monday–Friday 9:00 AM–4:00 PM). The pay range is $23.00–$25.00 per hour, DOE. Benefits include health, dental, vision, PTO, 401(k) match, HSA, tuition reimbursement, and more.

Qualifications

• 1+ years of experience in case management.
• 2+ years of social services experience, preferably in a senior care setting.
• Bachelor's Degree in social work or related program such as gerontology, psychology, or recreation therapy.
• Social Services certification preferred.

Responsabilités

• Assess residents' needs based on admission social services history, residents' health, psychological and social needs.
• Promote a positive living experience for our residents.
• Ensure confidentiality by adhering to HIPAA and all other privacy standards.
• Collaborate with community team members to support residents.
