Job summary

HumanGood in Burlington, NJ is hiring Social Services Coordinators to organize, implement and evaluate the Social Services program to meet residents' psycho-social needs. You will perform resident assessments, maintain records, and link residents with supportive services to help them live independently in the community.

The role is full-time with a 9:00 AM–4:00 PM schedule, and pay ranges from $23.00 to $25.00 per hour, depending on experience.

Qualifications

• 1+ years of experience in case management.
• 2+ years of social services experience, preferably in a senior care setting.
• Bachelor’s Degree in social work or related field.
• Social Services certification preferred

Responsibilities

• Assess resident needs based on admission social services history, health and psychosocial needs.
• Promote a positive living experience for residents.
• Ensure confidentiality by adhering to HIPAA and privacy standards.
• Collaborate with community team members to foster a positive work environment.
