Job summary

HumanGood is seeking Social Services Coordinators who are passionate, organized, and compassionate about residents. You will organize, implement, and evaluate the Social Services program to meet psycho-social needs, assess residents, maintain records, and ensure linkage to services to help them live independently in the community.

The role is full-time, Monday–Friday, 9:00 AM–4:00 PM, with pay range of $23.00–$25.00 per hour depending on experience, and a comprehensive benefits package is

Qualifications

• 1+ years of case management experience required.
• 2+ years in social services, preferably in a senior care setting.
• Bachelor's Degree in social work or related field.
• Social Services certification preferred.

Responsibilities

• Assess resident needs based on admission social services history, resident's health, psychological and social needs.
• Maintaining confidential resident records and ensuring HIPAA/privacy standards.
• Link residents to specific supportive services to help them continue living independently in the community.
