HumanGood is seeking a Social Services Coordinator to organize, implement and evaluate the Social Services program to meet psycho-social needs of residents. The role includes resident assessment, maintaining records, and ensuring residents are linked to supportive services to living independently in the community.

The position is full-time, with Monday to Friday from 9:00 AM to 4:00 PM. Compensation ranges from $23.00 to $25.00 per hour, depending on experience.

Qualifikationen

• 1+ year of case management experience required.
• 2+ years of social services experience, preferably in a senior care setting.
• Social Services certification preferred

• Assess resident needs based on admission social services history, health, and psychosocial needs.
• Promote a positive living experience for residents.
• Ensure confidentiality by adhering to HIPAA and privacy standards.
• Collaborate with community team members to support residents' well-being.
