HumanGood is seeking a Social Services Coordinator to organize, implement and evaluate the Social Services program, addressing psycho-social needs to help residents live independently in the community. Full-time schedule Mon-Fri 9:00 AM - 4:00 PM, with a pay range of $23.00-$25.00 per hour depending on experience.

The role requires a Bachelor's Degree in social work or related field, with 1+ years in case management and 2+ years in social services, preferably in a senior care setting.

Formación

• 1+ years of experience in case management required.
• 2+ years of social services experience, preferably in a senior care setting.
• A Bachelor's Degree required in social work or related program such as gerontology, psychology, or recreation therapy.
• Social Services certification preferred

Responsabilidades

• Assessing resident needs based on admission social services history, resident's health, psychological and social needs
• Promoting a positive living experience for our residents
• Ensuring confidentiality by adhering to HIPPA & all other privacy standards
• Promoting a positive work environment by collaborating with community team members
