Job summary

HumanGood is seeking a Social Services Coordinator to organize, implement, and evaluate our Social Services program for residents, helping meet psycho-social needs and support independent living in the community. You will perform resident assessments, maintain confidential records, and coordinate linkages to appropriate supportive services.

The role supports a full-time schedule in Glenside, PA with a focus on compassionate, collaborative care.

Qualifications

• 1+ years of experience in case management.
• 2+ years of social services experience, preferably in a senior care setting.
• Bachelor's Degree in social work or related program such as gerontology, psychology, or recreation therapy.
• Social Services certification preferred

Responsibilities

• Organize, implement and evaluate the Social Services program to meet the psycho-social needs of residents.
• Conduct resident assessments and maintain confidential records.
• Ensure residents are linked to the specific supportive services they need to live independently in the community.
