Job summary

HumanGood is seeking a Social Services Coordinator to organize, implement, and evaluate the Social Services program for residents. You will assess psycho-social needs and maintain records to connect residents with needed supportive services, enabling independent living in the community.

The role is full-time, with a Monday–Friday schedule from 9:00 AM to 4:00 PM. The pay range is $23.00-$25.00 per hour, dependent on experience, and benefits are available to eligible employees.

Qualifications

• 1+ years of experience in case management
• 2+ years of social services experience, preferably in a senior care setting
• Bachelor's Degree in social work or related program such as gerontology, psychology, or recreation therapy
• Social Services certification preferred

Responsibilities

• Assess resident needs based on admission social services history, resident's health, psychological and social needs
• Promote a positive living experience for our residents
• Ensure confidentiality by adhering to HIPAA and all other privacy standards
• Promote a positive work environment by collaborating with community team members
