Job summary

Socket.dev seeks a Service Coordinator to manage and coordinate support for individuals living in Agency properties or their own homes. You will assess needs, develop individualized service plans, and connect residents with community resources and services.

You will serve as a liaison among property management, residents, families, service agencies, and case managers, ensuring accessible services and effective partnerships while maintaining documentation and reporting per funder requirements.

Qualifications

• Associates degree preferred with 2 years work experience
• Ability to obtain certification within first year and join AASC
• Strong verbal and written communication is essential
• Proficiency in Microsoft Office Suite (Word, Excel, Outlook)
• Strong decision making and critical thinking abilities
• Ability to analyze information, evaluate needs and implement solutions
• Excellent organization and time management in a flexible environment
• Ability to establish and maintain relationships with staff, partners, and the public
• Ability to work independently and as part of a multidisciplinary team
• Ability to communicate professionally and respectfully

Responsibilities

• Assist individuals in homes, community, or clinic settings and explain program purposes
• Conduct assessments to identify factors affecting health and wellbeing
• Collaborate with participants to develop individualized health and social plans
• Coordinate intake activities, visits, assessments, and documentation
• Help residents access community and healthcare resources and schedule appointments
• Facilitate communication between healthcare providers, agencies, and families
• Maintain knowledge of community resources and best practices in service coordination
• Build trusting relationships with participants, partners, and team members
• Promote wellness and coordinate educational programs for residents and staff
• Assist residents in developing informal support networks
• Coordinate programs with service organizations for onsite or offsite services
• Maintain a directory of community resources and network with providers
• Distribute informational materials and ensure compliance with funding guidelines
• Document plans, activities, and outcomes per agency policies and funder requirements
• Participate in data collection, reporting, and evaluation of program outcomes
