Job Summary

The Case Manager works with a human service team to develop and execute service and care plans for residents. The role provides a supportive and empowering environment that promotes stabilization, independence, self‑reliance, and skills development. Responsibilities include executing and updating service plans, coordinating direct services and referrals, and monitoring resident behaviors and practices.

Pay: $24.04 an hour (approx. $50,004 annually)
FLSA: Non‑exempt. Eligible for overtime.
Schedule: Sunday–Thursday 9 a.m.–5 p.m. (Friday and Saturday off); or Tuesday–Thursday and Saturday 8 a.m.–4 p.m., Friday 10 a.m.–6 p.m. (Sunday and Monday off)
Location: Rego Park, Queens, NY

Job‑related Duties – Essential Functions
Service and Care Coordination
  • Ensure residents receive appropriate quality services per program requirements.
  • Design and coordinate service and care plans, including counseling, crisis intervention, advocacy, and linkages to external services.
  • Assist residents with immigration, citizenship, benefits, employment, and long‑term stability.
  • For family sites, coordinate transportation and educational services for children.
  • Develop and maintain a network of service referrals.
  • Meet with residents at least once a week to assess progress and modify plans.
  • Conduct weekly room inspections and coordinate with treatment teams to address safety and health concerns.
  • Design discharge plans to maintain enrollment in public benefits and community services.
Benefits / Entitlement Coordination
  • Enroll residents in eligible public benefits and city programs in accordance with DHS requirements.
  • Ensure residents complete required appointments, documentation, and activities for ongoing benefits.
  • Coordinate with immigration agencies and advocacy organizations to enroll residents in immigration services.
General Duties
  • Document interactions and communications with residents and in CARES and AWARDS systems.
  • Perform other duties as assigned by the supervisor or Program Director.
Qualifications

Education and Experience:
High School Diploma or GED required; bachelor’s degree in social work or psychology preferred. Three years of experience in a shelter or related field may satisfy the education requirement.

  • At least two years of experience in case management, counseling, or direct advocacy for clients in social service settings.
  • Experience with immigrants and immigration issues is a plus.
  • Excellent verbal and written communication skills; proficiency in English required; Spanish or other multilingual proficiency is a plus.
  • Basic computer skills in MS Word, Excel, Outlook, and internet use.
  • Ability to create simple financial budgets.
  • Physical ability to make unit visits, walk distances, and climb stairs.
  • Strong reasoning ability for prioritizing and decision‑making.
  • Experience engaging homeless individuals and people with health, mental health, or substance use challenges.
