Job summary

The Staff Speech-Language Pathologist at Canyon Vista Post Acute evaluates and treats residents, educates families, and maintains documentation in medical records. You will collaborate with physicians and health team members and provide ongoing training to facility staff.

Responsibilities include developing individualized treatment plans, supervising CFY students, coordinating discharge planning, and ensuring compliance with regulations while promoting resident safety and quality of care.

Qualifications

• Bachelor’s degree in Speech-Language Pathology; Masters/Doctorate preferred.
• Licensed Speech-Language Pathologist in the state; CE requirements expected.
• Proficient in electronic medical records and facility software (Net Health, PCC).

Responsibilities

• Evaluate residents to determine speech therapy needs and rehab potential.
• Develop and update individualized treatment plans to improve functional independence.
• Supervise CFY Speech Clinical Fellows and students.
• Communicate progress and discharge planning with families and physicians.
• Complete timely, accurate documentation in compliance with regulations.
• Educate residents, families, and caregivers on goals and home programs.
