Job summary

Rehab Centre in Mesa Hills Post Acute seeks a Speech-Language Pathologist to evaluate residents and develop individualized treatment plans. The role includes educating residents and families on goals and home exercises, and coordinating care with physicians and the health team.

Qualifications

• Bachelor’s degree in Speech-Language Pathology; Master’s or Doctorate preferred.
• Proficient computer skills with Net Health and Point Click Care.
• Licensed Speech-Language Pathologist in the state; maintain licensure and CE requirements.

Responsibilities

• Evaluate residents to determine speech therapy needs and rehab potential.
• Develop and update individualized treatment plans.
• Educate residents and families on treatment goals and home exercises.
• Supervise CFY Speech Clinical Fellows and students.
• Complete timely documentation and maintain records per policy.
• Coordinate with physicians and care teams.
