Job summary

Mesa Hills Post Acute is seeking a Speech-Language Pathologist to evaluate and treat residents, communicate with families, and maintain comprehensive documentation. The role includes training facility staff and guiding CFY students as needed.

The incumbent will collaborate with physicians and an interdisciplinary team to develop individualized plans, monitor progress, and participate in care conferences while ensuring regulatory compliance and quality of care.

Qualifications

• Bachelor’s degree in Speech-Language Pathology; Master’s or Doctorate preferred.
• Proficient in Net Health and Point Click Care.
• Licensed Speech Therapist in state; continuing education per state requirements.

Responsibilities

• Evaluate residents to determine speech therapy needs per physician orders.
• Develop and update individualized treatment plans to improve function.
• Supervise CFY and students as applicable.
• Communicate progress and discharge planning to families and medical staff.
• Document accurately per facility and payer requirements.
