Job summary

Qualifications

• High school diploma or equivalent; associate or bachelor’s degree preferred.
• Prior health-care experience preferred.
• Ability to read, analyze, and interpret regulations and technical procedures.
• Ability to write reports and business correspondence.
• Ability to present information and respond to questions from managers and employees.
• Mathematical skills with fractions, percentages, ratios and proportions.

Responsibilities

• Assist the Social Service Director in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration, medical and nursing staff in planning social services.
• Develop and coordinate department policies and procedures.
• Review department policies annually and participate in changes.
• Coordinate social service activities with other departments as necessary.
• Assist in discharge planning and coordinating community resources.
• Provide information to residents/families about Medicare/Medicaid and financial assistance.
• Maintain confidentiality of resident care information.
