Job summary

PACS Group is seeking a Social Services professional to assist planning, developing, organizing, implementing, evaluating, and directing our facility’s social service programs to meet residents’ medically related emotional and social needs. You will collaborate with administration, medical staff, and other departments to ensure comprehensive resident care.

Responsibilities include policy development, annual reviews, discharge planning, resource coordination, and guidance on Medicare/Medicaid

Qualifications

• Ability to read, analyze, and interpret general business materials.
• Ability to write reports, business correspondence, and procedures.
• Ability to present information clearly to managers and staff.
• Strong mathematical and analytical skills for practical tasks.
• Ability to solve problems with concrete variables and interpret written instructions.

Responsibilities

• Assist Social Service Director in planning, organizing, implementing, evaluating, and directing social service programs.
• Coordinate with administration, medical, nursing and other departments for social services planning.
• Develop and coordinate department policies and procedures; review annually.
• Identify residents' medically related social and emotional needs and plan accordingly.
• Provide information on Medicare/Medicaid and other financial assistance programs.
