Job summary

PACS in New York seeks a proactive Activity Coordinator to plan, develop, organize, and direct resident-centered activities across the facility. You will ensure programs meet residents’ interests while complying with federal and state regulations.

You will communicate with staff, residents, families, and external agencies to coordinate services, review surveys, and support discharge planning, outings, and in-room activities.

Responsibilities

• Plan and direct resident-centered activities for the facility.
• Maintain compliance with federal and state regulations and professional standards.
• Communicate with staff, residents, families and agencies to meet residents’ needs.
• Assist in planning community activities and facility surveys.
• Develop activity plans, discharge planning, and resident assessments.
• Arrange transportation for residents as needed.
• Create monthly activity schedules including outings and in-room activities.
• Provide reading materials including Braille or audio books as appropriate.
• Ensure progress notes document services and resident responses.
