Job summary

PACS Group is seeking an Activity Assistant to support the Activity Director in delivering a creative and interactive activities program that aligns with federal, state, local and corporate standards. This role focuses on assessing resident needs and planning engaging activities that meet physical, mental, and psychosocial needs.

Responsibilities include scheduling, coordinating transportation, documenting progress, and maintaining clear communication with residents, families, and staff.

Qualifications

• Ability to read technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Ability to effectively present information to managers and staff.
• Prefer experience in long term care settings.

Responsibilities

• Participate in planning and conducting of individual, small and large group activities.
• Assist in providing good communication between employees, residents, families, and public.
• Participate in community planning related to facility interests.
• Assist in development of monthly activity calendar and attendance records.
• Assist QA/Assessment Committee with action plans to address quality deficiencies.
• Participate in discharge planning and activity care plans and assessments.
• Assist in arranging transportation for residents as needed.
• Assist in developing monthly activity schedule for residents including outings and in-room activities.
• Encourage residents to participate in self-initiated activities and provide reading materials as needed.
• Assist with assessment documentation and quarterly progress notes.
• Keep Activity Department clean, orderly and secured.
