Job summary

PACS is seeking an Activities Assistant in a long-term care facility to plan and conduct individual and group activities, fostering open communication among staff, residents, families and external partners, and supporting the monthly activity calendar and attendance records.

You will assist the Activities Director with quality improvement, discharge planning, transportation arrangements and in-room activities for bed-bound residents.

Qualifications

• High school diploma or equivalent; prefer one year in long-term care.
• Reading and comprehension of technical procedures.
• Ability to read and comprehend policy and procedure manuals.
• Ability to present information clearly to managers and employees.

Responsibilities

• Plan and conduct individual and group activities for residents.
• Assist in maintaining communication among staff, residents, families and external partners.
• Assist in developing monthly activity calendars and attendance records.
• Support the Quality Assurance and Assessment Committee with action plans.
• Assist in discharge planning and development of activity care plans.
• Arrange transportation for residents for outings, appointments, or discharge.
• Encourage resident participation in self-initiated activities and hobbies.
• Keep the Activity Department clean, orderly and secured.
