The position requires a high school diploma or equivalent and preferable Activity Director certification, with experience in long-term care settings and strong collaboration with staff, residents, and families.

Qualifications

• High school diploma or equivalent required or better.
• Activity Director certification preferred.
• Familiarity with federal, state and local regulations for care facilities.
• Experience coordinating resident activities and staff communication.
• Ability to plan and evaluate activity programs.
• Strong attention to documentation and reporting.
• Knowledge of safety and accessibility considerations in activities.
• Commitment to resident-centered care and person-centered planning.

Responsibilities

• Plan, develop, organize, implement, evaluate, and direct resident-centered activities.
• Keep up-to-date with federal and state regulations and professional standards.
• Communicate effectively with staff, residents, families, and agencies.
• Coordinate community planning related to facility services.
• Participate in surveys and develop plans of correction for deficiencies.
• Assist in meetings and quality improvement initiatives.
• Discharge planning and development of activity care plans.
• Arrange transportation for residents as needed.
• Develop monthly activity schedules including outings and in-room activities.
• Encourage resident participation in self-initiated activities and provide materials (Braille/audio).
• Ensure complete and informative charted progress notes.
