Job summary

PACS is seeking a Social Services professional to assist in planning, developing, organizing, implementing, evaluating, and directing our facility’s social service programs to meet residents’ medically related emotional and social needs.

You will work with administration, medical staff, and other departments to develop policies and coordinate services, with focus on resident independence and well-being. Prior healthcare exposure is preferred.

Qualifications

• Must possess a high school diploma or equivalent.
• Associate or Bachelor’s Degree preferred but not required.

Responsibilities

• Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs.
• Meet with administration and staff to plan social services.
• Develop and coordinate department policies and procedures.
• Prepare and maintain regular social service progress notes.
