Job summary

PACS is seeking a Social Services Assistant to support planning, development, and direction of social service programs for residents. You will collaborate with administrators, medical staff, and families to address medically related social and emotional needs.

The role requires a high school diploma or equivalent, with Associate or Bachelor’s preferred. You will help implement policies, maintain records, and coordinate with community agencies to ensure quality resident care.

Qualifications

• Ability to read, analyze, and interpret general regulations and policy manuals.
• Strong written and verbal communication with residents, families, and staff.
• Ability to collaborate with multiple departments and external agencies.

Responsibilities

• Assist in planning, developing, and directing social service programs.
• Meet with administration and clinical staff to coordinate services.
• Develop and administer department policies and procedures.
• Maintain regular social service progress notes and reports.
• Coordinate discharge planning and community resources for residents.
