Job summary

Avina Healthcare is seeking a detail-oriented, empathetic Resident Services Coordinator to manage onboarding, administrative operations, and the care transition pipeline for our residents. You will serve as the primary liaison between families and our internal clinical team.

The role emphasizes meticulous data handling, timely communication with discharge planners, and coordination across care teams in a fast-paced environment. Training is provided for healthcare tech and EHRs.

Qualifications

• 1–3 years of experience in a long-term care, assisted living, or skilled nursing facility (SNF) setting.
• Experience using modern healthcare tech, CRMs, or EHRs; ExaCare preferred but training provided for strong digital literacy.
• Exceptional verbal and written communication skills for interacting with hospital discharge planners, families, and clinicians.
• Ability to multitask in a fast-paced environment while maintaining meticulous data accuracy.
