Job summary

Vantage Care, LLC is seeking an Admissions Coordinator to manage the end-to-end admissions workflow in a healthcare setting. You will coordinate with clinical, financial, and administrative teams to ensure smooth bed assignments and compliant documentation.

The role emphasizes strong communication, organization, and attention to detail, with daily updates, sign-ins, and PASRR completion prior to admission. On-site collaboration with leadership is required.

Qualifications

• No prior SNF experience required.
• Experience with PCC or similar EMR systems is highly preferred.
• Excellent verbal and written communication; able to interact professionally with families and staff.
• Strong organizational and time-management skills.
• Compassionate, resident-focused approach with transitions.

Responsibilities

• Email daily bed availability updates to the facility team and referral liaisons.
• Attend the morning meeting to review admissions, discharges, and bed moves with leadership.
• Bring updated face sheets for all admissions to Unit Managers during the morning meeting.
• Conduct unit rounds to ensure rooms/beds are prepared for new admissions or tours.
• Follow up on discharges within a 30-day window (48 hours then day 20).
• Verify that admission documentation is uploaded into PCC.
• Schedule equipment rentals from recent discharges and notify tracking.
• Manage and support the full admissions workflow, including PASRR completion and bed assignment.
• Ensure PASRR is completed before admission and Sign-Ins are recorded within 48–72 hours.
• Upload all Sign-In documents into PCC and keep Tour Packets updated.
