Mason Health Care Center in Cincinnati, OH is seeking a dedicated Social Services Designee to support residents and families. This full-time role focuses on admission, care, and discharge planning, providing emotional support and resources to residents and their loved ones.

You will document accurately, ensure compliance with state and federal requirements, and collaborate with nursing and therapy teams to promote quality care and a positive living environment for residents.

Qualifications

• Experience in a skilled nursing or long-term care setting preferred.
• Strong communication, organizational, and teamwork skills.
• Compassionate, resident-focused, and detail-oriented individual.
• LSW license not required — we will train the right candidate with a passion for working with seniors.

Responsibilities

• Coordinate and assist with admission, care, and discharge planning for residents.
• Provide emotional support, resources, and guidance to residents and families.
• Maintain accurate documentation and ensure compliance with facility, state, and federal requirements.
• Work closely with nursing, therapy, and interdisciplinary teams to ensure smooth transitions and quality care.
• Foster a positive environment that promotes residents’ well-being, adjustment, and quality of life.
