Ohio Living is seeking a Social Services Designee to plan, organize, implement, and evaluate social services for residents and patients in a life plan community. You will serve as a liaison to community resources and healthcare providers, ensuring emotional and psychosocial needs are met.

The role involves counseling, discharge planning, and coordinating care transitions, with emphasis on advocacy and timely information sharing across departments to improve resident outcomes.

Qualifications

• High school diploma or equivalent required.
• Associate degree in a related field preferred.
• If serving as Community Health Worker, must have completed Ohio Board of Nursing CHW training or be willing to within a reasonable timeframe.

Responsibilities

• Assist with planning, organizing, implementing, and evaluating social services for residents/patients.
• Provide information and referrals to appropriate agencies.
• Counsel residents/patients regarding plans to meet needs and maximize resources.
• Coordinate social services activities with other departments and community resources.
• Complete social histories, forms, and care plans in the EMR system.
• Assist with insurance concerns and advocate for residents' rights.
