Job summary

Monarch Healthcare Management seeks a Social Services Designee to support residents and families through admission and discharge by coordinating referrals, screenings, tours, approvals, and documentation while promoting facility services. This role builds relationships with referral sources and participates in the interdisciplinary care team.

Qualifications

• Strong communication, organization, management, and leadership skills.
• Experience in healthcare, skilled nursing facility or assisted living community.
• Ability to perform essential functions with or without accommodation.
• Must qualify for employment after a criminal background study per Minnesota DHS guidelines.

Responsibilities

• Conduct social assessments and mental health screenings as part of the care team.
• Prepare and maintain required MDS/CAA documentation and care plans.
• Assist with admissions, discharges, referrals, and family communications.
• Maintain accurate records and ensure regulatory compliance.
• Contribute to quality improvement efforts and policy development.
