Job summary

Monarch Healthcare Management seeks a Social Services Designee to support residents and families through admission and discharge, coordinating referrals and documentation while marketing facility services and fostering referral relationships.

You will conduct social assessments, psychosocial support, and required MDS/CAA documentation to develop individualized care plans that maximize residents’ functioning.

Qualifications

• Must recognize and understand the psychosocial and emotional needs of residents.
• Experience in healthcare, skilled nursing facility or assisted living community.
• Must have strong skills in communication, organization, management, and leadership.
• Must be able to perform essential functions of the position with or without reasonable accommodation.
• Must qualify for employment after a criminal background study, per Minnesota Department of Human Services guidelines.

Responsibilities

• Conduct social assessments and mental health screenings.
• Manage referrals, screenings, tours, approvals, and documentation during admission and discharge.
• Market facility services and build relationships with referral sources.
• Develop individualized care plans to promote each resident’s highest level of functioning.
• Maintain accurate records and ensure regulatory compliance.
• Uphold professional communication and confidentiality standards; participate in quality improvement.
• Contribute to policy development to support residents’ rights and well-being.
