Job summary

Forest Hills Healthcare Center in Cincinnati is seeking a Social Services Designee to support residents' psychosocial wellbeing and advocate for their needs in a long-term care setting.

As a full-time Day Shift position, you will collaborate with direct care staff, report grievances, maintain confidentiality, and coordinate with the activity team to involve families. Ohio LSW licensure is preferred; HS diploma is required; LTC experience is a plus.

Qualifications

• Must hold a Social Worker license (LSW) in Ohio or be eligible for licensure.
• High school diploma or GED is required.
• Prior social services experience in a long-term care environment is required or preferred.

Responsibilities

• Develop and maintain a good working rapport with direct caregivers to support residents’ psychosocial well‑being and program participation.
• Report grievances and provide oral/written reports to the Director of Social Services.
• Act as resident advocate and coordinate with care staff to meet residents’ needs (dress, appearance, etc.).
• Monitor changes in residents’ condition and report to the RN Charge Nurse.
• Provide information to the Director of Social Services to help resolve resident issues.
• Improve quality of services and expand activity opportunities for residents.
• Engage family members and guests in activities with the activity department when possible.
• Maintain confidentiality of all resident/family information.
