Qualifications

• Education: High school diploma or equivalent; an associate's degree preferred.
• Experience: Prior healthcare or social services exposure preferred.

Responsibilities

• Assist residents and families with emotional, social, and practical concerns.
• Help residents adjust to the facility and address social issues.
• Conduct interviews to assess needs, preferences, and concerns.
• Offer referrals to community resources such as counseling services and financial assistance.
• Assist with discharge planning and transportation arrangements.
• Collaborate with the care team to develop and implement care plans.
• Maintain confidential records and document interactions in the EHR system.
