Job summary

LionStone Care in Cincinnati, OH is seeking a Social Services Assistant to support residents and families with emotional, social, and practical needs within a healthcare setting. This role partners with the Social Services Director and care teams to ensure coordinated care and smooth discharge planning.

Qualifications

• Education: High school diploma or equivalent required. An associate’s degree or coursework in social work, human services, or a related field is preferred.
• Experience: Previous experience in a healthcare, long-term care, or social services setting is preferred.

Responsibilities

• Assist residents and families with emotional, social, and practical concerns.
• Help residents adjust to the facility and address care-related issues.
• Conduct interviews to assess residents' needs, preferences, and concerns.
• Provide referrals to community resources and financial assistance programs.
• Assist in developing discharge plans with the care team.
• Collaborate with the Social Services Director and care team to coordinate care.
• Maintain accurate and confidential records in the EHR system.
