Job summary

Star of Hope Mission in Houston, TX is seeking a Part Time Care Coordinator to support residents through the Star of Hope Resident Journey. You will provide ministry support, assess needs, develop goals, and coordinate services to promote healing, restoration, and self-sufficiency.

The role requires strong communication, documentation in CaseWorthy, and the ability to work with staff across departments in a Christ-centered setting. A B.A./B.S.

Qualifications

• B.A./B.S. in social services or related field with one year of related experience.
• Experience with homelessness or formerly homeless individuals preferred.
• Valid Texas driver's license with acceptable driving record and current liability insurance.
• Detail oriented, well-organized, and able to maintain accurate resident case files.
• Strong verbal and written communication; conflict resolution and public speaking.

Responsibilities

• Manage the resident life cycle from enrollment through progression, transition, or exit with service plans and support.
• Assess resident needs, barriers, and service needs; complete enrollments, exits, and updates in CaseWorthy.
• Meet with residents weekly to develop, monitor, and update service plans and progress.
• Connect residents to counseling, education, workforce development, medical care, and other supports.
• Maintain accurate documentation in CaseWorthy and communicate updates to management.
• Support program meetings, trainings, and collaboration across staff and volunteers.
• Support or teach program classes, ministry activities, and restorative programming in a Christ-centered setting.
