Job summary

Hope Haven is seeking a Service Coordinator to act as the internal case manager for adults, developing person-centered plans, leading cross-functional teams, and advocating for people served. The role includes coordinating enrollment teams, ensuring assessments, and maintaining comprehensive service plans.

You will lead the support teams, arrange funding and paperwork with regulatory bodies, and serve as the main contact for families and case managers, ensuring services align with individual

Qualifications

• Education must align with human services; leadership or coordination experience preferred.
• Ability to apply knowledge of mental, physical, and social impairments.
• Strong communication and computer skills; willingness to meet health and background requirements.

Responsibilities

• Receive referral packets and coordinate enrollment.
• Lead the enrollment team across service areas.
• Develop and maintain person-centered service plans and behavior/support plans as needed.
• Schedule and lead support team meetings; ensure timely reviews and documentation.
• Serve as liaison with family, case managers, and other stakeholders.
