Job summary

Path2Freedom seeks a Healthcare Coordinator to manage resident health needs across Safe Houses, coordinating appointments, meds, and documentation. This role requires travel between program locations and collaboration with providers, pharmacies, and the program team.

Ideal candidates are RNs with Florida licensure, strong organization and communication skills, and the ability to work independently within a trauma-informed, Christ-centered framework. A valid driver’s license is required.

Qualifications

• Registered Nurse (RN) preferred.
• Current Florida RN license preferred, if applicable.
• Experience in nursing, healthcare coordination, medication management, residential care, child welfare, or a related setting preferred.
• Experience working with children, adolescents, or young adults preferred.
• Must possess a valid driver's license and be able to safely transport residents.
• Proficiency with or ability to learn the following software and systems.
• Strong organizational and time-management skills.
• Excellent communication and interpersonal skills.
• Strong attention to detail and documentation accuracy.
• Ability to manage multiple appointments, medications, and follow-up needs.
• Ability to maintain confidentiality and exercise sound professional judgment.
• Ability to work independently and collaboratively within a multidisciplinary team.

Responsibilities

• Schedules, coordinates, and transports residents to medical appointments and maintains calendars.
• Documents appointment notes, provider recommendations, medication changes, and follow-ups.
• Coordinates prescription refills and serves as the primary contact for healthcare-related needs.
• Conducts initial health reviews and biweekly health check-ins with residents.
• Maintains each resident's healthcare/emergency records and required copies.
• Reviews medication administration documentation weekly and corrects gaps.
• Performs weekly medication counts and reconciles inventory.
• Trains staff in medication administration procedures and annual refreshers.
• Completes medication error reports and provides staff follow-up and education.
• Offers Christ-centered support and prayer when requested and appropriate.
• Maintains confidential records and ensures insurance access for residents.
