Shepherds House Ministries seeks a part-time Client Care Coordinator to support residents across sites. You will establish trusting relationships, assist with healthcare navigation, and coordinate care under the supervision of the RN Supervisor.

The role emphasizes trauma-informed, person-centered support, accurate documentation, and collaboration with nurses, case managers, and community partners. A high school diploma and Peer Support training are required, with experience serving vulnerable

Qualifications

• High school diploma or equivalent required.
• Completion of a recognized Peer Support training program, or ability to complete training within timeframe.
• Experience with homelessness, poverty, healthcare access barriers, behavioral health challenges, or similar barriers.
• Strong interpersonal and communication skills, including building trusting relationships and maintaining confidentiality.

Responsabilités

• Build trusting relationships with shelter residents with trauma-informed, person-centered engagement.
• Help residents identify healthcare needs and connect with services.
• Support residents with scheduling and attending healthcare appointments and referrals.
• Navigate insurance, benefits, transportation, and basic needs barriers for residents.
• Coordinate communication among residents, shelter staff, healthcare providers, and partners while maintaining confidentiality.
• Document resident contacts, referrals, services, and outcomes accurately.
